Me La Vang Community Health Services is committed to providing high-quality Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) services that support eligible adults in a homelike setting.

AFC/GAFC is a state-funded program designed to enhance quality of life and promote independence for individuals in need of support. The program serves adults aged 16 and older with physical, cognitive, or medical challenges who require assistance with personal care. Through AFC, participants receive compassionate, individualized care in a comfortable, home-like setting.


AFC/GAFC program offers older adults a safe, supportive, and compassionate alternative to institutional care. Designed to promote dignity and independence, AFC allows individuals to receive personalized assistance in a home-like environment while maintaining meaningful connections within their community.

Barbara Ly is an experienced Life Enrichment Coordinator dedicated to enhancing the quality of life for individuals in her community. With a strong background in long-term care, she specializes in planning and facilitating a wide range of activities designed to promote residents’ well-being, engagement, and overall happiness. Barbara takes pride in understanding each resident’s unique needs, interests, and abilities, ensuring that every program, social event, and individualized activity contributes to a meaningful and stimulating living environment.

Lina Nguyen has dedicated over 20 years to serving the community. Her extensive experience in Adult Day Health has provided her with deep insight into the needs of the older adult population. She is highly attuned to each individual’s unique personality, preferences, and psychosocial well-being.

As a certified medical interpreter specializing in the Vietnamese language, Lina bridges communication gaps with compassion and cultural sensitivity. Her prior role as an Admissions Coordinator in skilled nursing facilities has further strengthened her medical knowledge and understanding of healthcare systems. Lina is known for her professionalism, empathy, and unwavering commitment to improving the lives of others.

Joanna Ly is an experienced healthcare professional with over 20 years of practice across diverse clinical settings, including medical-surgical units, primary care, and urgent care. She holds a Master of Science in Nursing, specializing as a Family Nurse Practitioner, and brings a comprehensive understanding of patient care across the lifespan.

Known for her fun and energetic personality, Joanna loves being a provider and is deeply committed to delivering compassionate, patient-centered care. Her favorite aspect of being a nurse practitioner is the autonomy it provides and the ability to advocate for her patients. She takes pride in improving the health and well-being of the individuals and communities she serves.

Duc Ly is a proud U.S. Army Veteran and a registered nurse with over 15 years of experience in the healthcare field. He holds a Master of Science in Nursing from the University of Massachusetts Boston, specializing in Adult Gerontology.

Duc’s professional background includes extensive experience in community health, long-term care, and sub-acute rehabilitation, providing him with a comprehensive understanding of patient care across various settings. He is deeply committed to serving Veterans and older adults, bringing compassion, clinical expertise, and dedication to improving their quality of life.
